Q: How long can these toe separators be used?

As long as you need them to correct the problem without surgery.

Q: How do you know what size to order?

I don’t recall there being more than one size when I purchased these, and I don’t see a size on the box I purchased, though I suppose larger feet might need larger separators. I wear a size 9.5-10 women’s shoe, and if I were to need to decide on a size, I’d probably purchase the smaller size to try first. I use them between my big toe & the next one, on both feet, to keep them from pressing/rubbing together. If your toes are pressing into each other, my guess (considering my own experience using them) would be that it wouldn’t take too much to separate them. I hope this helps you with your decision!
